Fusion-4 is a new integrated multi-disciplinary, multi-agency service for disabled children, young people and their families. The core team includes Health, Social Services, Education and Voluntary sector staff. For each child, other agency staff involved with the child are included as part of the wider ‘team around the child’.
Who is the service for?
We provide a service for disabled children and young people aged between 0-19 years with complex needs and their families and carers. It covers the Mansfield area but is intended to influence the way services are provided across Nottinghamshire in the future.
What are the aims of Fusion-4?
- Better integration of services provided by the different agencies.
- Improved availability of information for young people, carers and workers.
- The involvement of disabled children and young people, their carers and families in the planning and delivery of services.
What are the objectives of Fusion-4?
We aim to think differently and develop a flexible service through an open relationship between agencies and users of the service Our objectives include;
- a local single access point for services
- multi-agency joint planning
- assessments and services at an early stage of need
- reduced fragmentation of services and co-ordinated responses
- ensuring families are aware of services and how they can access them
- ensuring services are jointly commissioned and provided.

Keyworkers
A Keyworker approach is central to the work of Fusion-4. Key workers will be identified through the direct impact they or their services are having on the disabled child, young person and family. They will work closely, supporting them to identify their needs and help to co-ordinate the appropriate services.
The Keyworker will be expected to empower disabled children, young people and their families, encouraging them to be involved as much as possible in planning and decision making. Key workers will be responsible for making sure that services are co-ordinated and regularly reviewed.
Every child, young person and family is different and therefore has different needs – we offer a flexible and tailored response to individual needs. All services and resources will be fully inclusive and will identify individuals or families who may be at risk of being marginalised or isolated for any reason.
