Service user reference group

The service user reference group had their first meeting on 3 March 2009 in Mansfield. The group plan to meet three or four times a year, until October 2010 when the project finishes.

Assessment questionnaire

The title for the questionnaire that decides what support service users need, and what their Personal Budget will be is the 'Self Directed Support Assessment'.
