Gladys

Gladys had a bad fall when she was doing some cleaning at home.

Unfortunately, she broke her hip and had to go into hospital.

Her injury meant that she would have difficulty walking and doing certain things (such as cooking and cleaning) for herself when she left hospital. Gladys wanted to carry on living in her own home so the ward staff contacted us. We arranged to visit her to see what services we could provide to help her.

Our social worker carried out an assessment and it was determined that Gladys was entitled to homecare and meals at home services.

One of our occupational therapists visited Gladys a few weeks later to see how she was coping around the house. It was clear that Gladys would not recover sufficiently from the broken hip to manage the stairs. As her bedroom, bathroom and toilet are all upstairs the occupational therapist helped her to apply for a disabled facilities grant to have a stairlift fitted.

Jack and Mary

Jack recently had a stroke leaving him unable to work. He is unsteady on his feet and gets anxious on his own and with strangers. Mary, his wife, had to give up work to care for him full time. They were finding it increasingly difficult to make ends meet and Mary's health was starting to suffer. A neighbour suggested to Mary that she contact us for help.

The social worker who visited Jack and Mary arranged for Nottinghamshire Welfare Rights Service to visit. The advice worker went through Jack and Mary's finances and helped them to claim extra benefits that they were entitled to.

The social worker also carried out a community care assessment which showed that that Jack was entitled to ten hours of homecare support a week. It was agreed that this would be provided as a direct payment so they could employ someone at their own convenience who Jack was familiar and comfortable with.

Mary also had a carer's assessment which resulted in her getting a regular break from her caring role while someone else cared for Jack.

The social worker was also able to provide information on local organisations that could offer other support opportunities.
