Future meeting dates;

All meetings will be held at High Oakham School

• Tuesday 10 January 2012 – 2.30 - 4.30
• Tuesday 7 February 2012 – 2.30 – 4.30
• Tuesday 6 March 2012 – 9.30 – 11.30

The CAF form is the referral form for a discussion at a JAT meeting. To initiate a discussion at a JAT you must complete at least the first two pages of the CAF (see right hand column) along with the consent section (although best practice would be to complete the whole form).

Actions agreed at the JAT meeting should be recorded on the CAF Review/Action Plan (see right hand column).

If you would like to discuss a child or young person at the JAT please complete the CAF form (see right hand column) and return to the JAT chair/business support admin one week in advance of the meeting.
