The decision to issue a statement of Special Educational Needs
If the LA decides to write a statement, you will receive a letter with a proposed statement attached.
The Proposed Statement
This document is in six parts
- Part 1 – gives your child’s name, date of birth, address and other family details. It also lists the reports we have received.
- Part 2 – describes your child’s Special Educational Needs
- Part 3- describes how your child can be helped in their school or setting
- Part 4- will describe the type and name of school when this has been decided
- Part 5 – describes any non-educational needs your child might have
- Part 6- describes the help that is provided by Health, Children’s Services or other agencies.
At the back of the statement, there is a list of all the different assessment reports and information that were used to write the statement. Each report that provides advice is called an appendix.
